Required Waiver for World Cup Soccer Camps

Parental Consent

I grant my child(ren) permission to participate in the World Cup Soccer Camps/Classes (WCSC). I understand that soccer activities are of a strenuous nature that do pose some risk of injury. As parent/guardian, I waive all claims of liability against World Cup Soccer Camps, its directors, employees and the cities, schools and facilities of Atherton and Laurel School (LC & UC) and Las Lomitas School, Belmont and Cipriani ES, Burlingame, Campbell, Cupertino and Far East Institute, Danville, Dublin, Foster City, Lafayette, Los Altos, St. Simon School and Los Altos School District, Town of Los Gatos, LGS, and Blossom Hill ES, Daves Ave ES, Lexington ES, Louis Van Meter ES, Menlo Park and La Entrada MS, Millbrae, Milpitas, Oceanside, Orinda and Orinda Intermediate School, Palo Alto and Bowman School, Redwood City, Reno and Swift Sportsdome, Rochester and Total Sports Experience, Rancho Cordova and Le Five Rancho Cordova LLC, San Carlos, San Bruno, San Francisco and City College of San Francisco, San Jose, Orion Montessori School and Saint Frances Cabrini School, San Mateo, Santa Cruz, Bay View ES, DeLaveaga ES, and Westlake ES, Saratoga and Argonaut ES, Saratoga ES, Foothill ES, Sunnyvale, Tustin, and Union City. My child will participate in the camp using proper protective equipment and he/she does so with my permission at his/her own risk.

In case of emergency, I hereby consent to medical examination and/or treatment for my child(ren). WCSC reserves the right to use any photographs or video taken by WCSC during camps, clinics, personal training or any other sponsored events by WCSC without the expressed written permission of those included within the photographs/video. WCSC may use the photographs/video in publications or other media material produced, used or contracted by WCSC including but not limited to: brochures, invitations, books, newspapers, magazines, television, websites, social media etc. Any person desiring not to have their photo taken or distributed must contact us at camps@worldcupsoccercamps.com in writing of his/her intentions.

By registering for our camps/classes/practices you confirm compliance with each county’s health guidelines.

City of Millbrae Waiver

I wish for my child to participate in a recreational activity and represent and agree as follows on behalf of myself and my child (collectively Participant):

I am the parent or legal guardian of the above named child.
Participant’s participation in the Activity is not required or recommended by the City of Millbrae (City) and is purely voluntary.
Participant should seek the advice of personal physician before participating in the Activity.
Participant agrees to abide by the rules issued by the City regarding the Activity, as they may be modified from time to time.
Participant could become injured during the Activity. Participant is aware that participation in the Activity may involve risk of injury, adverse medical condition, loss or damage to Participant or Participant’s property or other consequences, including, without limitation, contraction of COVID-19. These risks of injury, loss or damage might result from Participant’s own actions or negligence, or the actions or negligence of others, or the conditions of the premises or of any equipment used. Participant acknowledges that there may be other risks to Participant that are not known or not reasonably foreseeable. Participant agrees to assume all of the above risks.
Participant also releases, waives, discharges, and promises not to sue the City or its councilmembers, employees, officers, agents, affiliated companies, and insurers from any and all liability for injury, adverse medical condition, loss or damage to Participant’s person or property, or any other consequence in connection with participation in the Activity.
Participant also agrees to indemnify, defend and hold harmless the City, its councilmembers, employees, officers, agents, affiliated companies, and insurers from any and all damages, losses, liabilities, fees and costs incurred in connection with any third party claims or threats of claims asserted arising from or relating to Participant’s participation in or conduct associated with the Activity.
Participant intends this release and hold harmless agreement to forever bind Participant as well as Participant’s estate, personal representatives, guardians, conservators, heirs, executors, administrators, next of kin, and assigns.

By clicking ‘I AGREE’ I represent that I have read, understand, and consent to the terms of this waiver and release on behalf of myself and my child.
